Insurance Coverage Options

Currently offering in-network coverage for most of the following:

• Anthem PPO plans

• Anthem BCBS of Indiana

• Blue Cross Blue Shield PPO plans

• Cigna

• Multiplan - most PPO plans, plans that have the Multiplan logo

• PNOA - must have PNOA logo

• UHC-UMR - commercial plans

• VA CCN - Optum East (veterans only, no spouses or dependents, referral required)

The Lactation Network 

Those who wish to utilize this option can go through my link with The Lactation Network. Once approved, TLN will never send a bill to a client. Credit card on file with me will not be charged except for in the event of a late cancellation without rescheduling. Clients will receive an EOB from their insurance, this is not a bill. TLN never bills clients once they've obtained pre-approval. Reach out to TLN for specific questions concerning billing.

Wildflower 

Wildflower works specifically with Cigna Health insurance. If your Cigna plan works with Wildflower, you can obtain pre-approval through my link at Wildflower. Once pre-approved, Wildflower will never send you a bill. Again, your EOB is not a bill.

Private Pay Clients may make payment through credit card, Venmo, or Paypal. A superbill will be provided to all private pay clients upon request. Credit card on file will be charged the day of your visit prior to the visit starting. 

*Payment for services are required in advance. Pre-approval with TLN or Cigna, or credit card payment due the day of services.

*Credit card on file required of all clients.

*Cancellations with less than 24 hour notice will be subject to $50 cancellation fee.

*Military Discounts for active duty parents and their spouses insured under military insurance plans that do not qualify under The Lactation Network or Wildflower

*Lactation Care is Health Care and a right. Cost should not be the prohibitive factor. Please reach out if you need support service options forwarded to you.
